http://socialinnovation.typepad.com/silk/ Since September SILK have been speaking to professionals, front line workers, people living with dementia and their carers to understand the dementia care pathway from their point of view. People have been honest and open about sharing their experiences, letting us know where things are working well and where things could be changed and improved. The Insight Report summarises what we have learned while working on this project, with direct quotes from the wide range of people we have spoken to who are living and working with dementia day-to-day. While we have included the main themes and issues that have been brought to our attention by multiple people during various conversations, there were of course many other individual experiences that we have not been able to include. People also shared with us their ideas and suggestions of what could have been improved for them and how they feel things could be improved for others in the future.

Le mouvement de Transition est né en Grande-Bretagne en septembre 2006 dans la petite ville de Totnes. L'enseignant en permaculture Rob Hopkins avait créé le modèle de Transition avec ses étudiants dans la ville de Kinsale en Irlande un an auparavant.
