Sometimes there'd be young kids on the train commuting to school, but their heads would be too low for the pusher to see amngst the crowd, so it would appear to him like there was a gap - i.e., more space. So he'd keep shoving.
One kid I saw get smushed in so hard he cried. ...And no one said anything. I've never been, but yes, Japan, at least in its modern media, seems a little weird.
