In recent years, the glaring spotlight of skepticism has been turned to the topic of vaccines, bitterly dividing parents and giving doctors of modern medicine conniption fits — but no matter where you stand on vaccinations, the fact two groundbreaking, scientific, peer-reviewed studies painting inoculations in not-so favorable light have vanished from the Internet is cause for concern. Originally, the study, titled, “Pilot comparative study on the health of vaccinated and unvaccinated 6- to 12- year old U.S. children,” was published in Open Access Text. Until, that is, Open Access Text removed it and a second, similar study by the same authors from their archives without explanation.

2. What are the risks? In the modern era, it is rare to suffer permanent disability or death from measles in the United States. Between 1900 and 1963, the mortality rate of measles dropped from 13.3 per 100,000 to 0.2 per 100,000 in the population, due to advancements in living conditions, nutrition, and health care—a 98% decline (Fig. 1).2,5 Malnutrition, especially vitamin A deficiency, is a primary cause of about 90,000 measles deaths annually in underdeveloped nations.6 In the U.S. and other developed countries, 75–92% of hospitalized measles cases are low in vitamin A.7,8

There were on average 50 and 121 times more pages in the clinical study reports than in their corresponding trial register entries and journal publications. This was likely a main reason why the clinical study reports were superior at reporting trial design aspects. If our systematic review of clinical study reports [9] had relied on trial register entries or journal publications, it would have had no data for a quarter of our prespecified outcomes (11/40). Although the inclusion of clinical study reports led to significantly more eligible and available data, no changes in the direction of available results occurred when comparing the risk ratios of corresponding meta-analyses as ratios of relative risks. This may have several explanations. First, GlaxoSmithKline might be more transparent than other pharmaceutical companies [36], so corresponding study documents from GlaxoSmithKline could be more consistent compared to corresponding study documents from other companies [37,38,39,40].

The Promise of mRNA Vaccines
Earlier this month, the world finally received some good news about COVID-19. Interim results from Phase 3 clinical trials revealed that two vaccine candidates —one from the Pfizer and BioNTech and another from Moderna—were more than 90 percent effective. In addition to sharing what appears to be very high efficacy, the vaccines have something else in common: they are both made with messenger RNA (mRNA). mRNA vaccines work by providing the genetic code for our cells to produce viral proteins. Once the proteins, which don’t cause disease, are produced, the body launches an immune response against the virus, enabling the person to develop immunity. mRNA can theoretically be used to produce any protein, with the upside that it much simpler to manufacture than the proteins themselves or the inactivated and attenuated versions of viruses typically used in vaccines, making it an appealing technique, says Norbert Pardi, an mRNA vaccine specialist at the University of Pennsylvania.
Computer Simulations to Enhance Vaccine Trials
Infectious disease emergencies are opportunities to test the efficacy of newly developed interventions—for example, drugs, vaccines, and treatment regimens. Yet they raise many intertwined challenges around politics, logistics, ethics, and study design. It is essential to advance the discussion of how such products can and should be tested while remaining consistent with the efforts of CEPI, WHO, and others who encourage development and testing of candidate vaccines in advance of emergencies. This can help disentangle ethical from political and logistical concerns, reduce the time pressure to make a decision, and encourage rational deliberation by future stakeholders who at the time of deliberation do not know what role (which product, which field site) they may be supporting in an actual emergency.
