PLOS 29/04/16 Underestimation of Leptospirosis Incidence in the French West Indies

As in many other countries, under-reporting of cases was suspected in the French West Indies because of inadequate access to diagnostic tests for the general population. Methodology/Principal findings In order to estimate the real incidence of leptospirosis in Guadeloupe and Martinique, a study was performed in 2011 using the three prevailing available biological tests for diagnosis: Microscopic Agglutination Test (MAT), IgM ELISA and PCR. Conclusion/Significance Inclusion of PCR and IgM ELISA tests for diagnosis of leptospirosis resulted in improved sensitivity in comparison with MAT alone. Author Summary Leptospirosis is a common disease in tropical regions around the world. It is caused by Spirochetes of the Leptospira genus and affects all mammals. The WHO considers it as a re emerging disease because of global warming. About 500 000 severe human cases are recognized worldwide but the incidence is probably under evaluated because of the diagnosis difficulties. Metropolitan France is the most affected European country with about 300 human cases declared per year. Infection results from exposure to infected urines of carrier animals, either directly or indirectly via contaminated environment. PLOS 03/11/15 An Unprecedented High Incidence of Leptospirosis in Futuna, South Pacific, 2004 – 2014, Evidenced by Retrospective Analysis of Surveillance Data

The first human leptospirosis case was confirmed in 1997. Active surveillance started in 2004. Cases were confirmed by PCR or real time PCR, or by serology using MAT or a combination of IgM-ELISA and MAT. A retrospective analysis of surveillance data shows that the disease was endemic with a mean annual incidence of 844 cases per 100,000 over an 11-year period from 2004 to 2014. However, very little is known about the circulating etiological agents of leptospirosis in this region. In this study, we describe the serological and molecular features of leptospires isolated from 104 leptospirosis patients in Guadeloupe (n = 85) and Martinique (n = 19) and six rats captured in Guadeloupe, between 2004 and 2012. Methods and Findings Strains were studied by serogrouping, PFGE, MLVA, and sequencing 16SrRNA and secY. Conclusions The overall serovar distribution in the French West Indies was unique compared to the neighboring islands. Among the 45 participants who were contacted, 41 returned a completed questionnaire, of whom eight met the outbreak case definition. The eight cases sought medical attention and were given antibiotics within the first week after fever onset. No severe manifestations of leptospirosis were reported. In seven of the eight cases, the infection was confirmed by qPCR. Three pathogenic Leptospira species, including L. kmetyi, were identified in four of the cases. Introduction Over the past few decades, sporting events in tropical areas have become increasingly popular among travellers and athletes. Leptospirosis is a bacterial zoonosis, distributed worldwide, whose incidence is higher in the tropics, where transmission conditions are favourable. Methods Results.
