Administering a class survey to each student is an effective way to receive targeted feedback at the end of the school year. Here is a format teachers can follow to create their own class survey. Create a Survey Structure A student survey can be as simple as five open-ended questions or a more complex two-page evaluation of a course. Whatever the teacher decides to create, she should first determine the overall purpose of the survey and then design a structure to fulfill this purpose.

You might also like to think about developing a similar survey for the beginning of next year to give some insight into student’s attitudes that they bring with them to your class. You could include the various learning areas, what they are expecting to gain from the year with you, and what questions they would like to find out more about.
