MEDICAL CORPS INTERNATIONAL FORUM. Physician associate (physician assistant) What: A clinical role under the supervision of a doctor who is responsible for the care of patients and tailors the physician associate’s scope of practice according to their skills and the patient profile. The role was developed in the United States in the 1960s to address workforce shortages. The Netherlands, Canada and the UK have recently adopted the role. Australia has also run trials, and as a result has started a training programme at a Queensland university. Aims: For physician associates to serve in a complementary role, working alongside doctors, nurses and others. Physician associates can work across traditional vocational boundaries but always under the supervision of an appointed doctor. The flexibility to perform a variety of tasks that complement medical and nursing teams is valuable where there are skill shortages.

Training: New Zealand medical schools have expressed an interest in offering physician associate training. PA Forum. CMC hosted a forum in March to discuss extended nursing roles and physician assistants and the how these roles work in the multidisciplinary team. An official position statement has since been developed. Des Gorman rejects nursing criticism that HWNZ’s innovations have been poorly planned and taken a fragmented approach. He is particular proud of the diabetes nurse specialist prescribing innovation (initiated by the multidisciplinary NZ Society for the Study of Diabetes under the championship of nurse practitioner Helen Snell) as a successful backdoor entry point for nurse prescribing in general. “The diabetes nurse prescriber is the biggest thing by far that’s extended the nursing role for probably the last decade or two,” says Gorman.

“The failure to get nurses – short of nurse practitioners – the prescribing role has been because my profession has simply blocked it.” He said the diabetes innovation succeeded because the potentially disrupted group, the doctors, were strongly in favour of an extended nursing role. “Why has nurse prescribing not been on the books a long time ago?” He asks rhetorically. Physician assistants inevitable Gorman clearly sighs when the topic is brought up.
